Osteoarthritis: management strategies depending on the location of lesions

Osteoarthritis (OA) is a multifactorial disease that can be caused by genetic factors, obesity, joint microtrauma, and excessive physical exercises. The key features of OA include gradual loss articular cartilage, bone tissue remodeling, development osteophytes, subchondral sclerosis. pathogenesis based on the glycosaminoglycans, collagen disorganization, inflammation mediated cytokine cascade. complex affecting not only but also intraarticular periarticular tissues. Together, these changes compromise function cause chronic pain. Considerable attention has beer recently paid to phenotyping in order introduce tailored approach into patient management. Phenotypes are applicable three most common peripheral locations: gonarthrosis, coxarthrosis, hand OA. An international group experts primary care developed algorithms for management, including diagnostic criteria, treatment options, criteria referring specialist (rheumatologist). Current guidelines recommend non-drug therapy, pharmacotherapy, surgery. There growing interest phytopharmaceuticals, heterogeneous molecules with high capacity suppressing inflammation, oxidative stress, pain, as well improving function. Mucosate® capsules (DIAMED-pharma, Russia) one currently available products phytocompounds. It contains NEM® (natural eggshell membrane), Harpagophytum root extract, glucosamine sulfate, chondroitin manganese sulfate monohydrate.
